Key Responsibilities
Coordinate the interdisciplinary plan of care from admission to discharge while integrating patient and family preferences. Monitor patient progress through assessments and record reviews to ensure appropriate service delivery and effective communication among the healthcare team.
Requirements
Requires a Registered Nurse credential from the Illinois Board of Nursing and a diploma from an accredited nursing school. Previous inpatient discharge experience is highly desired for this role.
