Key Responsibilities
The Registered Nurse Case Manager oversees patient care plans, coordinates multi-disciplinary services, and performs skilled nursing visits in patient homes. They are responsible for accurate clinical documentation, medication administration, and supervising nurse aides and LPNs.
Requirements
Candidates must hold a current Registered Nurse license and have at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required for travel to patient locations.
