The Nurse Practitioner will provide hospice and palliative care to seriously ill patients in their homes to alleviate symptoms and align treatments with patient goals. They will also collaborate with a clinical team and provide ongoing education to the medical community within their territory.
Candidates must be a graduate of an accredited nursing program and possess at least 2 years of experience in chronic disease management or long-term care. A valid driver's license and reliable transportation are required, along with the physical ability to lift 80-100 lbs.
