Best Hospice Care of Texas is looking for a compassionate and dependable Full-Time RN Case Manager to join our team!
As an RN Case Manager, you’ll manage a full patient caseload while working closely with patients, families, physicians, and our interdisciplinary team to provide compassionate, individualized hospice care.
What You’ll Do
- Manage and coordinate care for a full patient caseload
- Complete skilled nursing visits and ongoing patient assessments
- Develop and update individualized plans of care
- Provide education, guidance, and support to patients and families
- Collaborate with physicians and the interdisciplinary hospice team
- Maintain timely and accurate clinical documentation
- Participate in IDT meetings and on-call rotation as assigned
- Coordinate changes in patient condition and ensure continuity of care

Hospice or Home Health Experience Preferred

Requirements
Requires a current Texas RN license, compassion, dependability, organization, strong clinical assessment and communication skills, and the ability to independently prioritize a full caseload. Hospice or home health experience is preferred.
