Job Description
Responsible for organizing and providing care to patients through the process of assessment, planning, intervention, and evaluation.
Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers.
The role integrates and coordinates utilization management, care facilitation and discharge planning functions. This role is accountable for planning effectively in order to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload.

- Current RN licensure in appropriate state(s) of employment
- Basic Cardiac Life Support (BCLS) certification within 1 month
- Fargo: Basic Cardiac Life Support (BCLS) not required

Key Responsibilities
Organize and coordinate patient care through assessment, planning, intervention, and evaluation, supporting the physician and interdisciplinary team throughout the care continuum and discharge process. Coordinate utilization management, care facilitation, and discharge planning while managing patient needs, length of stay, resource use, clinical outcomes, and timely, accurate payer information.
Requirements
Requires a BSN or ADN from an accredited nursing school or college and current RN licensure in the appropriate state or states of employment. BCLS certification is required within one month, except in Fargo, where it is not required.
