Your Impactful Role as a Physical Therapist
In Your Role, You Will
- Provide One-on-One Care: Evaluate and treat patients in their homes, developing individualized treatment plans focused on mobility, function, independence, and quality of life.
- Manage Your Schedule: Enjoy the autonomy of managing your patient visits while traveling throughout your assigned service area.
- Help Patients Reach Their Goals: Use your clinical expertise to help patients improve strength, balance, mobility, and functional independence.
- Collaborate with the Care Team: Work closely with nurses, occupational therapists, aides, social workers, physicians, patients, and families to coordinate comprehensive care.
- Educate Patients and Caregivers: Provide education and instruction on therapeutic exercises, mobility, safety, and strategies that support continued progress at home.
- Document and Communicate: Maintain timely, accurate clinical documentation and communicate patient progress and changes in condition to the appropriate members of the care team.

What You'll Need to Excel
- Licensure: A current Minnesota Physical Therapist license.
- Education: Graduate degree from an accredited Physical Therapy program.
- Patient-Centered Mindset: A genuine passion for helping patients achieve their goals and maintain independence.
- Clinical Expertise: Strong assessment, treatment planning, and clinical decision-making skills.
- Independence: The ability to work confidently and effectively in a home health environment.

- Driver's License: A valid driver's license and reliable transportation.
