Your Impactful Role as an RN Case Manager 

In your pivotal role, you will

* Manage and Schedule Visits: Enjoy the independence of managing your patient visits, traveling within the community to provide personalized, one-on-one nursing services in patients’ homes. This role allows you to make deep connections with patients, ensuring they receive the utmost care and attention.

* Interdisciplinary Collaboration: As a vital part of our interdisciplinary team, your leadership will enhance our collaborative, supportive environment, ensuring the highest well-being and care standards for our patients.

What You'll Need to Excel 

* Licensure: A valid Minnesota Registered Nurse license or Minnesota/North Dakota compact license.
