Job Description

Key Responsibilities
The RN Case Manager will coordinate care for hospice patients, including performing nursing assessments and developing individualized plans of care. They will also provide skilled nursing services focused on symptom management and collaborate with an interdisciplinary team to support patients and families.
Requirements
Candidates must hold a current Minnesota Registered Nurse license and have graduated from an accredited nursing school. Preferred qualifications include a Bachelor's degree in Nursing and two years of recent experience in hospice, home health, or acute care settings.
