Key Responsibilities
The RN Case Manager will conduct nursing assessments, develop individualized care plans, and coordinate all aspects of hospice care for assigned patients. They will also collaborate with an interdisciplinary team and provide education and support to patients and their families.
Requirements
Candidates must be a graduate of an accredited school of nursing and hold a current Minnesota Registered Nurse license. Preferred qualifications include a BSN and two years of recent experience in hospice, home health, or acute care nursing.
