Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical Jacksonville - Downtown campus.
This is a Full Time Day shift opportunity working Monday - Friday in the In-Patient Case Management Department.
Case Manager, Job Responsibilities
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
Experience Requirements
1 year Case Management experience
Education & Credential Requirements
- Associate's Degree Nursing (ADN)
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- FL Active licensed Registered Nurse (RN)

Key Responsibilities
The Registered Nurse Case Manager is responsible for coordinating care for hospitalized patients and performing high-risk screenings and assessments. They also manage the coordination of post-acute care services while collaborating with a multidisciplinary team.
Requirements
Candidates must hold an Associate's Degree in Nursing and an active Florida Registered Nurse license. Additionally, the role requires one year of case management experience, Basic Life Support certification, and a valid driver's license.
