- Schedule: Monday-Friday| 8:00 AM - 4:00 PM

Community Care, Inc. is looking for a compassionate and dedicated RN to provide home health care to our clients in the Westmoreland and Fayette County area. What You'll Do
Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments)
- Develop and implement individualized care plans
- Educate patients and caregivers on managing conditions and medications
- Collaborate with a multidisciplinary team to ensure continuity of care
- Document visits and patient progress accurately and timely
What You'll Need
- Current Registered Nurse (RN) license in the state of Pennsylvania.
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience (Home Health experience preferred)
- CPR certification (required)
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for pediatric care

Requirements
Applicants must hold a current Pennsylvania RN license, graduate from an accredited nursing program, and have at least one year of clinical nursing experience; home health experience is preferred.
