Job DetailsJob Location: Bethany Children's Health Center - Bethany, OK 73008Job Shift: Indirect CareThe Clinic Nurse Navigator serves as a patient advocate, interpreter, educator and counselor for our outpatient clinic patients. Under limited direction, this position functions on the multidisciplinary team to coordinate patient care throughout the continuum of care and serves as a clinical resource for care management. Duty Percent of Time Cultivates and supports primary care and specialty provider co-management with timely communication, inquiry, follow-up and integration of information into the care plan regarding transitions in care and referrals. 25% Serve as the contact point, advocate and informational resource for patient families, care team, and sub-specialists. 10% Increases continuity of care by managing relationships with ambulatory care providers, transitions in care, referrals, and authorizations. 10% Communicates information and participates in satellite clinics, multidisciplinary clinics, and everyday operational functions of outpatient clinic. 15% Directly involved with the development and enhancement processes of multidisciplinary specialty clinics with the aim to improve the clinical experience for patients and families. 35% Other duties as assigned. 5% Qualifications Education: Bachelor's degree in health-related field Experience: 3+ years clinical experience; Pediatric experience preferred License/Certification/Credential: Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state About Us Our work begins where we are all equal - at the heart. Requirements
