Job description
Job Title: Registered Nurse Navigator - Breast Imaging
Department: Breast Imaging
Location: Imaging Center - Rochester General
Hours Per Week: 40 hours
Schedule: Monday through Friday; Days; 8:00AM - 4:30PM
SUMMARY:
The Breast Imaging Navigator will work within a medical interdisciplinary team to provide proactive case coordination, navigation, and education for patients in need of breast care treatment, including case management, utilization management and resource management for advanced imaging studies. The Breast Imaging Navigator will work under the direction of the radiologist and collaborate with the breast care center, oncology and referring providers to provide comprehensive and multidisciplinary breast care for the patient and family. The breast-imaging navigator follows health system policies and procedures, and assists with continually improving the quality and effectiveness of image case management and patient navigation.
RESPONSIBILITIES:
Serve as patient navigator for breast imaging patients to include mammography, breast MRI’s, breast ultrasounds, biopsies/procedures, and follow up care.
Provides professional guidance to our patients in coordination with our women’s breast center.
Main point of contact for referring provider or patient questions/scheduling breast procedure add-ons.
Assists with scheduling biopsies, reviewing consent for biopsy/procedures with patient prior to procedure, assist with procedures as needed.
Responsible for the communication to patients regarding follow up imaging needed based on the radiologist request.
Schedules appointment for additional imaging based on radiologist recommendation prior to patient being referred back to surgeon as appropriate.
Maintains appropriate documentation through medical record entries, MQSA data base and navigator dashboard, daily logs, computer entries and monthly statistics in a timely manner.
Revamp patient scheduling template to accommodate add on surgical procedures from breast center and to validate open procedure spots not utilized are filled with other exams.
Provides High quality, patient centered clinical care.
Performs initial and ongoing assessment of the patient, utilizing appropriate data (i. e. physical, psychological, educational).
Demonstrates clinical judgment in responding to priority patient needs.
Communicates and documents assessment of patient with the interdisciplinary team, utilizing all available resources in order to meet the needs of the patient and family.
Demonstrates ability to manage job related stressors and constructive feedback during intra/interdepartmental relations.
Participates in problem solving activities in the breast imaging unit
Serve as a patient navigator for breast imaging patients to include mammography, breast MRI’s, breast ultrasounds, biopsies/procedures and follow up care.
Assist with scheduling biopsies, reviewing consent for biopsy/procedures with patient prior to procedure, assist with procedures as needed.
Contact patient pre-procedure to review medications, identify patient is able to withstand prone procedure during stereotactic biopsy.
Reviews and preps for interventional breast imaging procedures Reviews cases with the radiologists as needed.
Prepares and explains post procedure care with patients and their family.
Maintains appropriate documentation through medical record entries in EPIC, daily logs, computer entries and monthly statistics in a timely manner.
Follow up on patients post procedure care, to include directive of Radiologist on benign results.
Help to oversee pathology reports and follow up with radiologist on addendums made within one week.
Oversees weekly all BiRADS 4 & 5 validating follow up care is completed within a timely manner.
Initiates and documents education plan for the patient and family based on identified learning.
IV starts and contrast injection for Contrast Enhanced Mammography, monitor patient during CEM procedure.
Medical support for vasovagal and emergency events.
Supports and helps maintain our patient risk assessment program, assess to identify high risk patients.
Create weekly procedure list and procedure reference sheet for Radiologist – submitting in advance to them.
Consistently upholds and promotes the mission of Rochester Regional Health through living core values with the workplace.
May act as a liaison between Radiologist and staff Technologist.
Complies with organizational policies, procedures and protocols including policies governing safety,
medication administration and documentation
Responsible for compliance of departmental safety procedures and assists in training staff on new safety protocols as applicable.
Provides feedback to breast imaging supervisor/manager concerning daily issues and problems.
Demonstrates independence and strong clinical skills.
Orders supplies and maintains par levels as assigned.
Assists in DNV and DOH inspection compliance and training.
Performs or has extensive knowledge of the various Ultrasound, MRI and Mammo procedures including stereotactic.
Works with other departments to facilitate scheduling of procedures in a timely manner.
Works closely with Breast Imaging Supervisor and Imaging Navigator
Requires a strong working relationship with breast radiologist and the breast surgeon along with breast center staff.
Provides support, direction and assistance to the multiple ambulatory imaging sites that perform mammography.
Performs other duties as assigned.
REQUIRED QUALIFICATIONS:
Diploma or Associates Degree in nursing required.
NYS Registered Nurse license
Basic Life Support (BLS) Certification

Bachelor’s Degree in Nursing Preferred
1-3 years’ experience as a Registered Nurse preferred; demonstrated leadership qualities and behaviors preferred.

Light - Clinical - Light clinical roles involve frequent standing and walking, with occasional lifting of light objects (≤20 lbs.) and routine reaching, grasping, and fine manipulation for tasks such as patient care, laboratory work, or specimen processing. Staff rely on strong visual and auditory acuity to accurately observe specimens, read instrumentation, monitor patients, respond to alarms, and communicate effectively with care teams. Cognitive demands include multitasking, prioritizing clinical and laboratory workflows, and coordinating with interdisciplinary teams. Emotional resilience is necessary due to frequent interruptions, shifting priorities, and the need for precise and compassionate interaction with patients and colleagues. Environmental exposures may include infectious materials, biological specimens, cleaning agents, sharps, chemical reagents, and wet surfaces. PPE use, TB testing, and schedule flexibility are commonly required.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.

Requirements
Candidates must hold a Diploma or Associate Degree in nursing and possess a valid NYS Registered Nurse license. Basic Life Support (BLS) certification is required, with a preference for 1-3 years of nursing experience and a Bachelor's degree.
