Key Responsibilities

The Nurse Navigator serves as a patient advocate and care coordinator for individuals with cognitive or memory diagnoses. They provide clinical guidance and logistical support while collaborating with multidisciplinary teams to optimize patient outcomes.

Requirements

Candidates must hold an Associate’s degree in nursing and possess a current Registered Nurse license in Minnesota or Wisconsin. Additionally, two years of neurology nursing experience and current BLS certification are required.
