The Opportunity

Generation Clinical Partners (GCP) is a physician-led medical group caring exclusively for older adults living in skilled nursing and assisted living communities.

We were founded on the vision that frail older adults need clinicians who have time to understand their complex needs, who are consistently present where they live, and who respond quickly when a condition changes.

For more than a decade we have built our model around longer, more thoughtful visits and responsive onsite care — not high-volume, episodic medicine. We have performed well in value-based programs since 2015, from Bundled Payments to I-SNPs to Accountable Care Organizations.

We are now expanding into North Carolina and seeking a physician to work with an established skilled nursing operator in the Rocky Mount area, about an hour east of Raleigh. You will work closely with advanced practice providers (APPs) in this role.

Depending on your interests and availability, the position may also include medical direction. The commitment can sit anywhere along a wide spectrum — from a part-time addition to your current practice through to full-time with GCP — as either an independent contractor or an employee.

Why Physicians Join Us

You decide how your day goes. This is the part physicians unfamiliar with our practice find hardest to believe until they experience it. There is no clinic template, no assigned start time, and no one dictating which days you round.

You establish a predictable cadence with each community based on patient and facility needs, and beyond that the day is yours to build — when you arrive, how you sequence visits, when you finish. Our physicians schedule around school pickups, standing commitments, and in some cases even a second practice.

You practice in one setting, and you get good at it. No clinic-plus-hospital-plus-nursing-home juggling. You care for one population and develop real expertise in its functional, cognitive, and medical complexity.

Call is shared and it is by phone. No in-person call. No routine weekend or holiday rounding.

You are never the only one there. Our model pairs physicians with dedicated APP support, so patients are covered between your visits and you do not carry the panel alone.

You are close enough to the outcome to see it. You see the same patients over time, in the place they live, alongside the same nurses and families — so you are the one who notices when something changes and can act in time to achieve the best outcome possible.

You have administrative support. Credentialing, billing, and IT support are handled by our team so you can practice medicine. Our tools include athenaOne configured for post-acute workflows, voice dictation and ambient scribing, and OpenEvidence or UpToDate.

You work directly with GCP's founder. Walter Lin, MD, MBA serves on the boards of PALTmed, the professional society for post-acute and long-term care clinicians, and LTC ACO, a Medicare accountable care organization focused on long-term care. He provides in-person support during onboarding and remains available afterwards for ongoing training as needed.

The Work

Attending Physician

  • Provide onsite primary care, typically across 2–3 primary communities for a full-time physician: admission history and physicals, follow-up visits, and management of acute changes and chronic conditions
  • Lead advance care planning and goals-of-care conversations
  • Manage medications with attention to deprescribing and polypharmacy
  • Coordinate with facility clinical staff, families, and palliative care or hospice partners

Serve as clinical mentor and resource to the APP at each community — sharing complex and regulatory visits and raising the quality of care across the team

Maintain a predictable onsite presence, respond promptly to clinical concerns, and complete documentation, orders, and follow-up accurately and on time

  • Participate in GCP's care coordination, transitional care, and quality improvement programs
  • Help establish GCP's clinical culture and relationships in a new market, in partnership with GCP leadership
  • Medical Directorship (facility-dependent, additional stipend)

Some partner facilities ask GCP to provide medical director services.

Where an appointment is available and agreed, responsibilities include monthly QAPI participation and follow-through on improvement priorities; advising facility leadership on clinical policies; supporting survey readiness and regulatory compliance; staff education; and helping address medical-staff performance concerns through the facility's leadership process.

GCP provides training, mentorship, and ongoing support, including for first-time medical directors. There is an additional stipend associated with Medical Directorships.

Compensation and Benefits

For a full-time physician, total compensation can exceed $300,000, through a combination of:

Patient-encounter compensation based on clinical productivity

Non-patient-encounter compensation, including shared savings tied to value-based outcomes, medical director fees, and NP collaboration fees

This figure is achievable net of practice costs at sufficient clinical volume. Actual earnings depend on schedule, patient panel, productivity, quality and value-based performance, and additional responsibilities.

Eligible W-2 physicians can participate in group medical, dental, and vision insurance; a 401(k) with employer match; CME time; and professional development including clinical mentorship and leadership training.

Who May Be a Good Fit

We welcome physicians with SNF or medical-director experience, and equally those whose clinical work already includes substantial care of older adults. Prior senior-living experience is helpful but not required, and physicians interested in developing medical-direction skills are encouraged to discuss training and support.

Required: MD or DO; active unrestricted North Carolina license or eligibility to obtain one; board certified or board eligible in Internal Medicine, Family Medicine, Geriatric Medicine, or another appropriate specialty; DEA registration before starting; reliable transportation and ability to travel among assigned communities; eligibility to enroll and remain in good standing with Medicare, Medicaid, and applicable health plans.

Preferred: experience in skilled nursing, long-term care, assisted living, or geriatrics; familiarity with EHR documentation and charge capture; working knowledge of the CMS Five-Star Quality Rating System; experience in value-based or managed care arrangements.

Requires an MD or DO degree with an active, unrestricted North Carolina license and board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine. Candidates must have reliable transportation and the ability to travel among assigned communities.
