Job Details: Job Location: Seymour - Seymour, WI, Position Type: Full Time, Salary Range: Undisclosed, Join our team as a Rehab Director – Physical Therapist Greenfield Rehab is seeking an experienced and dedicated Physical Therapist (PT) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Seymour, WI.

The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility.

This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency.

The Rehabilitation Director also functions as a licensed Physical Therapist, providing direct patient care as needed. We are looking for leaders who: Ensure the delivery of high-quality, patient-centered rehabilitation services. Strong clinical reasoning and patient identification skills.

Experience managing medical complexity and changing functional status. Comfortable collaborating with nursing, therapy, and interdisciplinary teams. Encourages collaboration, values others' contributions, and builds trust.

Qualifications Graduate from an accredited school with a minimum of a bachelor's degree in physical therapy. Licensed by the State of Wisconsin to practice as a Physical Therapist.
