Job Description
Dove Healthcare - West Eau Claire | 1405 Truax Blvd Eau Claire, WI 54703 As our long-term care social worker, you will be part of an interdisciplinary team providing case management for our residents. You will advocate for resident rights, coordinate services, and help meet the psycho-social needs of our residents.
The Long-term Care Social Worker Is Responsible For
evaluating resident psycho-social needs completing a comprehensive assessment and developing a plan of care exercising independent judgment / making sound decisions /exhibiting flexibility when needed working knowledge of federal and state regulations ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to long-term care
Required Qualifications
bachelor's degree in social work or related human services field current certification in the State of Wisconsin as a certified social worker, and must maintain CEU's as per WI State Statute
Schedule Details
full-time: M-F, 8:00am-4:40pm no weekends or holidays
