Job Description
Job Summary
- Manages the case management team for optimal operation of care coordination.
- This is achieved by assisting the Director to create a culture of learning and
collaboration with the healthcare team while focusing on patient-centered care.
Contributes to overall hospital quality, customer satisfaction and financial
goals and ensures compliance with local, state and federal regulations and
accrediting organizations. The incumbent is responsible for departmental
management 24 hours a day, 7 days a week.

Experience
- Minimum Requirements
- RN with 5 years of clinical experience in acute care nursing; 2 years of case
management experience.

- Preferred/Desired: Certified CM
- Special Skills
Minimum Required

Requirements
Requires a BSN and RN license with at least 5 years of acute care nursing experience and 2 years in case management. Preferred qualifications include an MSN and CCM or ACM certifications.
