Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by collaborating with healthcare teams and families. This role involves managing individualized care plans and supporting patients through complex healthcare transitions while working autonomously in a field-based capacity.
Requirements
Candidates must hold an A.D.N. degree or higher and a current RN license, with at least five years of clinical nursing experience. Proficiency in case management, basic computer skills, and the ability to travel within the assigned territory are required.
