Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by collaborating with healthcare teams and families. This role involves managing individualized care plans and supporting patients through complex healthcare transitions while maintaining clinical and financial outcomes.
Requirements
Candidates must hold an A.D.N. degree or higher, a current RN license, and at least five years of clinical nursing experience. Proficiency in case management, renal nursing, and basic computer skills is required, along with a valid driver's license and home office setup.
