Key Responsibilities
Conduct initial client assessments and continuously refine care plans while providing hands-on nursing care. Manage patient care coordination and participate in interdisciplinary team meetings to ensure comprehensive care planning.
Requirements
Must be a licensed Registered Nurse (RN) in the state of practice with proficiency in Electronic Medical Records (EMR) software. Requires a valid driver's license, a reliable vehicle with insurance, and strong organizational and communication skills.
