Key Responsibilities
The RN Case Manager facilitates effective transitional care coordination by working with inter-professional teams to ensure continuity of care. They are responsible for developing individualized care plans, conducting holistic assessments, and ensuring compliance with regulatory standards.
Requirements
Candidates must be a graduate of an accredited nursing school with current RN licensure and at least three years of recent clinical experience. A Basic Life Support (BLS) certification is required, and a Bachelor of Science in Nursing (BSN) is preferred.
