RN Case Manager - Part-Time

Key Responsibilities

The RN Case Manager facilitates effective transitional care coordination by collaborating with inter-professional teams and developing individualized care plans. They are responsible for patient advocacy, conducting holistic assessments, and leading patient rounding activities to ensure continuity of care.

Requirements

Candidates must be a graduate of an accredited nursing school with a current RN license and at least 3 years of recent clinical experience. A BLS certification is required, and a BSN or relevant certifications like CCM or ACM are preferred.
