Key Responsibilities
Hospitalist Case Managers collaborate with interdisciplinary teams to manage patient care plans and facilitate smooth transitions at discharge. They are responsible for coordinating services, educating nursing staff, and ensuring all discharge documentation is completed accurately.
Requirements
Candidates must hold a Bachelor of Science in Nursing (BSN) and a valid Registered Nurse license in Kansas. A minimum of 5 years of acute care nursing experience is preferred, along with proficiency in discharge planning and clinical assessment.
