Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by collaborating with healthcare teams and providers. This role involves managing individualized care plans and supporting patients through complex healthcare transitions in a field-based capacity.
Requirements
Candidates must hold an A.D.N. or higher nursing degree, a current RN license, and at least five years of clinical nursing experience. Proficiency in case management, current CPR certification, and a valid driver's license are also required.
