Key Responsibilities
The Case Manager, RN conducts nursing assessments and develops personalized care plans as a key member of the interdisciplinary team. They also deliver nursing interventions, coordinate care with physicians, and educate participants and their families on disease management.
Requirements
Candidates must hold an unencumbered California RN license and have at least two years of clinical nursing experience with frail or elderly populations. A degree from an accredited nursing school is required, with a Bachelor of Science in Nursing preferred.
