Key Responsibilities
The Primary Care Provider will perform comprehensive in-person assessments and collaborate with an interdisciplinary team to develop and implement participant care plans. They are responsible for providing continuous medical coverage and overseeing care coordination for seniors in various settings.
Requirements
Candidates must be a licensed Nurse Practitioner or Physician Assistant with prescriptive authority and at least two years of experience working with elderly populations. A reliable means of transportation and the ability to participate in rotating night and weekend calls are required.
