Key Responsibilities
Perform comprehensive in-person assessments and lead an interdisciplinary team to develop personalized care plans for seniors. Coordinate discharge for acute, short-term, and long-term placements while providing holistic care.
Requirements
Requires successful completion of an accredited residency program and an active state physician license. Must hold or be eligible for DEA registration and state CDS certification, with board certification preferred.
