Key Responsibilities
Perform comprehensive in-person assessments and lead an interdisciplinary team to develop personalized care plans for seniors. Coordinate discharge planning for acute and long-term care while providing holistic, community-based medical services.
Requirements
Requires completion of an accredited residency program and an active state physician license without restrictions. Candidates must hold or be eligible for DEA registration and CDS certification, with board certification preferred.
