Key Responsibilities
The Case Manager RN guides participant care by conducting assessments and developing personalized care plans within an interdisciplinary team. They deliver nursing interventions, coordinate care with physicians, and educate participants and their families on disease prevention and self-care.
Requirements
Candidates must be a graduate of an accredited nursing school with an unencumbered California RN license and current BLS/First Aid certification. Two years of clinical experience with frail or elderly populations is preferred, and a BSN is desired.
