Work Environment

Community-based geriatric primary care serving residents in memory care, assisted living, and independent living communities throughout the Kingman area

  • Coverage Model: Independent advanced practice with physician support and collaboration available when needed
  • Travel: Local travel between assigned communities; reliable transportation required
  • On-Call: Not specified
  • About the Opportunity / A Day in the Life

This is an opportunity for an FNP or AGNP who wants meaningful patient relationships, clinical autonomy, and the ability to take ownership of their practice. You’ll provide primary care to older adults living in memory care, assisted living, and independent living communities throughout Kingman.

Your day will center on managing the full spectrum of geriatric primary care—from chronic disease and preventive care to acute concerns, medication management, and follow-up visits. Many patients are medically complex, requiring strong clinical judgment, patience, and the ability to balance multiple conditions while keeping quality of life and independence at the center of the care plan.

You’ll work with a high level of independence while maintaining access to physician support when needed. You’ll also collaborate closely with facility staff, nurses, therapists, specialists, patients, and families to coordinate care and address changes in health before they become larger problems.

For the right provider, this role offers the independence and ownership of building a patient panel and practice style without the administrative burden of owning a private practice.

Why This Role Stands Out

  • Meaningful continuity of care: Build lasting relationships with older adults and their families rather than seeing patients for isolated episodes of care.
  • High clinical autonomy: Make decisions and manage your patient panel with minimal micromanagement.
  • Flexible schedule: Structure your workday with greater flexibility than many traditional clinic-based roles.

Complex, engaging medicine: Manage a diverse geriatric population that will challenge and strengthen your clinical judgment.

Physician support without overmanagement: Practice independently while having collaboration and mentorship available when needed.

Opportunity to grow: Take ownership of your practice and develop professionally as the patient population and organization grow.

  • What We’re Looking For
  • A provider with a genuine interest in caring for older adults
  • Strong clinical judgment and comfort managing medically complex patients
  • An independent, accountable approach to patient care
  • Excellent communication with patients, families, facilities, and clinical teams
  • A thoughtful, patient-centered approach to treatment decisions
  • Someone who values autonomy while recognizing when collaboration is appropriate

Job Duties & Responsibilities

  • Provide comprehensive primary care to residents of memory care, assisted living, and independent living communities
  • Evaluate and manage chronic diseases, acute medical concerns, and medically complex geriatric conditions
  • Conduct preventive care, routine assessments, and follow-up visits
  • Develop individualized treatment plans focused on health, function, independence, and quality of life
  • Manage medications and identify opportunities to reduce unnecessary polypharmacy when clinically appropriate
  • Monitor changes in patient condition and adjust treatment plans accordingly
  • Collaborate with physicians, nurses, therapists, specialists, facility staff, patients, and families
  • Coordinate patient care across facilities, specialists, hospitals, and other healthcare settings
  • Educate patients and families regarding diagnoses, medications, treatment plans, and goals of care
  • Maintain accurate, complete, and timely documentation in the electronic medical record
  • Exercise independent clinical judgment while seeking physician collaboration when appropriate

Required Qualifications

  • Licensed Nurse Practitioner with FNP or AGNP preparation and eligibility to practice in Arizona
  • DEA registration or ability to obtain
  • Strong clinical decision-making and critical-thinking skills
  • Excellent interpersonal and communication skills
  • Ability to practice independently and collaboratively
  • Reliable transportation for local travel between facilities
  • Preferred Background
  • Two or more years of experience caring for geriatric patients
  • Experience in assisted living, memory care, long-term care, internal medicine, family practice, hospice, or palliative care
  • Experience managing medically complex older adults
  • Familiarity with medication management and care coordination in senior living environments

New graduates are encouraged to apply. Candidates should bring a genuine passion for geriatrics, a strong work ethic, eagerness to learn, and a commitment to providing exceptional patient care.

  • This Role Is Likely a Fit If You…
  • Have a genuine interest in serving older adults and improving their quality of life
  • Enjoy developing long-term relationships with patients and families
  • Want meaningful autonomy and ownership of your patient panel
  • Are comfortable making independent clinical decisions
  • Enjoy solving complex clinical problems rather than following a highly standardized workflow
  • Are dependable, organized, compassionate, and accountable for your patients' care
  • This Role May Not Be a Fit If You…
  • Prefer a traditional clinic environment where patients come to a single location
  • Are uncomfortable traveling locally between senior living communities
  • Prefer close day-to-day supervision or a highly structured clinical workflow
  • Are not comfortable managing patients with multiple chronic conditions and complex medication regimens
  • Prefer episodic care over developing ongoing relationships with patients and families

Why Join Us

This role offers the opportunity to practice geriatric medicine in a way that combines autonomy, continuity, flexibility, and meaningful patient impact. You’ll have the freedom to take ownership of your patient panel while still having physician collaboration and clinical support available when you need it.
