Family Nurse Practitioner / Physician Assistant – Adult Primary Care

Mesa, AZ | Adult-Only Outpatient | Monday–Friday | New Grads Welcome

This is a strong opportunity for an FNP or PA who enjoys relationship-based primary care and wants an adult-only outpatient practice with a predictable Monday–Friday schedule.

Your day will include a mix of routine primary care, wellness visits, follow-ups, acute concerns, and chronic disease management. You’ll evaluate patients, order and interpret appropriate testing, manage medications, develop treatment plans, and educate patients on their health and ongoing care.

Because this is an adult-only practice, you can focus your clinical work on adult and chronic disease management without a pediatric patient population. You’ll also have physicians, APPs, and experienced clinical staff around you for collaboration and support.

  • Job Duties & Responsibilities
  • Provide comprehensive outpatient primary care to adult patients
  • Perform new patient evaluations, follow-up visits, and preventive wellness visits
  • Diagnose and manage acute and chronic medical conditions
  • Develop individualized treatment and ongoing care plans
  • Order, review, and interpret laboratory and diagnostic testing
  • Prescribe and manage medications within scope of practice
  • Provide preventive care, health education, and lifestyle counseling
  • Monitor chronic conditions and adjust treatment plans as appropriate
  • Coordinate referrals and continuity of care with specialists and other healthcare providers
  • Collaborate with physicians, APPs, and clinical support staff
  • Maintain accurate and timely clinical documentation

Required Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program in the United States
  • Current or eligible for Arizona NP or PA licensure
  • FNP certification for Nurse Practitioner candidates or appropriate PA certification
  • Strong communication and interpersonal skills
  • Ability to provide professional, patient-centered outpatient care

Preferred Background

  • 2–3 years of Primary Care, Family Medicine, or Internal Medicine experience preferred, but not required
  • Previous outpatient experience is a plus
  • Experience managing common adult chronic conditions is beneficial
  • New graduate FNPs and PAs with a strong interest in adult primary care are encouraged to apply
