Location & Logistics
Work Environment
- On-site outpatient primary care clinic in Phoenix, AZ - no inpatient responsibilities
- Coverage Model or Hours: Full-time, normal business hours
- Travel: None - this is a single-site role
- On-Call: Call expectations, if any, are reviewed during the interview process
- About the Opportunity / A Day in the Life:
You will carry a full outpatient schedule of roughly 20-30 patients a day, working across the whole range of primary care: acute visits, chronic disease management, preventive screenings, annual wellness visits, and the follow-up that keeps a patient panel healthy between appointments. You assess, order and interpret diagnostics, build the treatment plan, and own the continuity that comes with seeing the same patients over time.
The patient population is diverse and community-rooted, and a good share of your visits will be conducted in Spanish. Being able to explain a diagnosis, a medication change, or a follow-up plan in a patient's own language is the difference between a visit that gets documented and a visit that actually changes an outcome - which is why bilingual English/Spanish ability is a requirement rather than a bonus here.
You will not be doing it alone. You work alongside physicians, medical assistants, registered nurses, and care coordination staff, with leadership that is reachable and a team culture built on backing each other up. Documentation runs through eClinicalWorks (eCW), so prior eCW experience shortens your ramp - though strong clinicians who have used other systems pick it up quickly.
Why This Role Stands Out
- Pure outpatient practice - no inpatient duties, no rounding, no hospital coverage
- Normal business hours and a predictable weekly schedule
- A real patient panel with continuity, not a revolving urgent-care door
- Your Spanish is a clinical tool here, used every day, not a line on your resume
- Supportive leadership and an experienced support team around each provider
- A growing outpatient medical group with room to grow as it expands
What We're Looking For
- An NP or PA with at least two years of primary care or outpatient experience
- Comfort managing both acute complaints and long-term chronic conditions independently, within scope
- Fluent, confident clinical communication in English and Spanish
- Someone who works well inside a team of physicians, nurses, MAs, and coordinators
- A provider who can hold quality and documentation standards at a 20-30 patient-per-day pace
- Genuine interest in serving a diverse, community-rooted patient population
Job Duties & Responsibilities
- Diagnose, evaluate, and manage acute and chronic medical conditions across a diverse patient population
- Deliver comprehensive primary care including preventive screenings, annual wellness visits, and patient education
- Conduct thorough patient assessments and develop evidence-based treatment plans
- Order and interpret diagnostic testing, and adjust treatment accordingly
- Manage approximately 20-30 patient visits per day while holding quality and satisfaction standards
- Prescribe and manage medications within your scope of practice
- Partner with physicians, medical assistants, registered nurses, and care coordination staff on shared patients
- Communicate clearly with English- and Spanish-speaking patients about diagnoses, treatment plans, and follow-up
- Maintain accurate, timely, and complete documentation in the electronic medical record
- Support chronic disease management and preventive health initiatives across the patient panel
- Coordinate referrals and follow-up care so patients do not fall through the gaps
- Build long-term patient relationships that keep people engaged in their own health
Required Qualifications
- Active Arizona Nurse Practitioner (NP) or Physician Assistant (PA) license
- Minimum of 2 years of clinical experience in primary care or an outpatient setting
- Bilingual proficiency in English and Spanish
- Strong clinical assessment, diagnostic, and treatment planning skills
- Experience managing acute and chronic medical conditions
- Excellent communication, interpersonal, and patient care skills
- Ability to work independently while collaborating effectively with a multidisciplinary clinical team
Preferred Background
- Experience working with diverse or underserved patient populations
- Experience with the eClinicalWorks (eCW) EMR
- Background in preventive medicine, chronic disease management, and patient education
- This Role Is Likely a Fit If You…
- You want outpatient primary care with continuity and no inpatient obligations
- You practice comfortably and confidently in both English and Spanish
- You like a busy clinic day and can keep your documentation current inside it
- You want colleagues and leadership who are actually available when you need them
- You are looking for a long-term home rather than a stop-gap assignment
- This Role May Not Be a Fit If You…
- You do not hold - or cannot obtain - an active Arizona NP or PA license
- You have less than two years of primary care or outpatient clinical experience
- You are not able to conduct patient visits in Spanish as well as English
- You are looking for inpatient, hospital-based, or procedure-heavy work
- You want a remote or telehealth-only position - this role is on-site in Phoenix
