Job Description

Job Summary
We are seeking a dedicated and compassionate Post Acute Primary Care Physician (MD/DO) to join our growing post-acute care team in Jacksonville, FL. In this role, you will perform daily rounds at designated Skilled Nursing Facilities (SNFs), ensuring high-quality transitional and long-term care for our residents. You will manage complex clinical cases, admit new patients, and provide vital clinical oversight to a collaborative team of Advanced Practice Providers (APPs).
Schedule: Full-time: 5 days per week
Hours: Monday to Friday, 8:00 AM – 5:00 PM
Key Responsibilities
- Conduct daily clinical rounds across assigned Skilled Nursing Facilities (SNFs) in the Jacksonville area.
- Evaluate and examine all newly admitted patients to establish comprehensive care plans.
- Directly manage higher-risk, medically complex patients requiring intensive monitoring.
- Provide clinical supervision, mentorship, and chart sign-offs for Advanced Practice Providers (APPs).
Qualifications & Requirements
Board Certified (BC) or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics (Board Certification is NOT a requirement).
- Active Florida Medical License in good standing.
- ACN (Area of Critical Need) license holders are accepted and welcome to apply.
- Strong communication skills and a passion for geriatrics/post-acute medicine.
Compensation & Benefits
- Base Salary: $225,000 – $245,000 per year
- Time Off: 2 weeks (14 days) of Paid Time Off (PTO)
- Insurance: Comprehensive Medical Malpractice Insurance included
- Professional Development: Continuing Medical Education (CME) allowance
Requirements
Candidates must hold an active Florida Medical License and be Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics. Strong communication skills and a passion for geriatric medicine are required.
