• Home Health RN Case Manager | $5,00 Sign On Bonus
• Boston Proper, Including Chinatown

Full-Time | $90,000–$115,000 Guaranteed Salary + Productivity Bonus | Paid Parking Pass

You’ll provide skilled care in patients’ homes and help them recover, manage their health, and stay independent. And because parking is part of working in Boston, we provide a paid parking pass for this territory.

What You’ll Do

You’ll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

• What You’ll Bring
• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout Boston Proper, including Chinatown
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
