Job Description
- STATUS: Full Time/Hybrid
- LOCATION: 3379 Chili Avenue, Suite 100, Rochester, NY 14624
- DEPARTMENT: Family Medicine
- SCHEDULE: Mon, Tues, Fri- in office & Wed, Thurs- remote; 8:00 AM - 4:30 PM
Position Summary
The primary therapist, senior, provides mental health diagnostic assessment, treatment planning, and mental health therapies for persons with mental illness/substance abuse. Treatment is provided in the context of a clinical team, supervised by the staff psychiatrist. Provide superior customer service.
Health Reach Only: The therapist will work within the Health Reach Health Care for the Homeless Program which incorporates both a central clinical site as well as outreach within the community. Treatment is provided in the context of a multidisciplinary clinical team.
Key Responsibilities
- Provides initial and ongoing assessment, counseling and clinical intervention to a designated caseload.
- Provides individual, group, and family psychotherapy.
- Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care, and communicates with relevant internal and external care providers.
- Participates in and incorporates professional supervision and ongoing education toward quality care
- Utilizes efficient computer skills to complete all required documentation in a timely and comprehensive manner.
- Maintains a full caseload, meeting productivity requirements and seeks to keep the incidence of failed appointments to a minimum.
- Productivity standards have been met over the past 12 months

- No disciplinary action over the past 12 months
- Ability to supervise interns
- Shows initiative (ie. supervision of an intern or; participation or leading a project team; process development, in charge of a committee in the program)
- Trains, coaches and mentors new therapists
- ACT Program Only: Primary resource for team on integrating family goals and services with the tasks of all team members.
- ACT Program Only: Provides individual and group family psycho-education.
- Health Reach Only: Will be an active member of the Health Reach Continuous Quality Improvement Committee and will participate in other program and community wide activities.
- For Child and Youth Services Only: Off-site and in-home delivery of services to clients and families.
Minimum Qualifications
- Masters Degree or PHD in Human Services field required.
- Two years of mental health experience preferred.
Required Licensure / Certification Skills
LCSW, LMSW, LMHC, LCAT, LMFT or earned doctorate in human services field required. Limited permit or license required within 6 months of hire.

PHYSICAL REQUIREMENTS:
Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments.
Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments.
These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery.
Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting.
Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.

The posted pay range is a good faith representation of potential base pay for this position. It may be modified in the future and eligible for additional pay components. Individual annual salaries/hourly rates could vary based on position details and FTE. Pay is determined by considering factors including, but not limited to, experience, relevant qualifications, specialty, internal equity, locations, and contracts.

Requirements
Candidates must hold a Master's degree or PhD in a human services field and possess a valid clinical license such as LCSW, LMSW, LMHC, LCAT, or LMFT. Two years of mental health experience is preferred, along with strong documentation and clinical assessment skills.
