Key Responsibilities
Provide neurobehavioral health services to residents in long-term care facilities using a multi-disciplinary approach. Collaborate with specialty clinicians to ensure continuity of care for elderly patients with neurocognitive and psychiatric conditions.
Requirements
Requires a Master’s degree in Mental Health Counseling or Social Work and current licensure as an LMHC or LISW. Candidates must be able to work within an interdisciplinary team and navigate electronic medical record systems.
