Position Summary As a Psychiatric Mental Health Nurse Practitioner (PMHNP) with Impact Health PC in Poughkeepsie, NY, you will provide psychiatric care to residents within skilled nursing facilities.

You will assess and manage behavioral health conditions, develop individualized treatment plans, manage psychiatric medications, and collaborate with facility staff, physicians, therapists, and interdisciplinary care teams.

Responsibilities

Provide person-centered psychiatric care using evidence-based practices. Conduct psychiatric assessments and diagnose behavioral health conditions. Develop individualized treatment plans and manage psychiatric medications. Manage an assigned caseload and provide ongoing psychiatric follow-up.

Provide psychoeducation and medication education to patients and families. Collaborate with facility staff, therapists, physicians, and interdisciplinary care teams. Complete timely and accurate clinical documentation. Maintain compliance with facility, state, and federal regulations.

About Us

Impact Health PC https://impacthealthpc.com Qualifications: Active Psychiatric Mental Health Nurse Practitioner (PMHNP) license in New York. Minimum 2 years of clinical experience providing mental health care to adult populations preferred.

Experience in skilled nursing facilities, geriatric mental health, hospitals, residential care, or outpatient psychiatric settings preferred. Proficiency with EHRs and clinical technology. Strong communication and interpersonal skills.
