Position Summary As a Nurse Practitioner (NP) or Physician Assistant (PA) with Impact Health PC in Monroe, CT, you will provide Medical/Primary Care to residents within Assisted Living Facilities (ALFs). You will serve as a primary care provider for your patients, managing both acute and chronic conditions, developing individualized treatment plans, and coordinating care with facility staff, physicians, specialists, families, and other members of the care team.

Key Responsibilities Provide comprehensive Primary Care to residents in Assisted Living Facilities. Assess, diagnose, and manage acute and chronic medical conditions. Perform routine evaluations and ongoing patient follow-up. Develop evidence-based, individualized treatment plans. Order and review appropriate medications, labs, imaging, and other diagnostic testing. Complete timely clinical documentation and place appropriate orders. Coordinate care with facility staff, physicians, specialists, families, and interdisciplinary care teams. Educate patients and families regarding conditions and treatment plans. Maintain compliance with facility, state, and federal regulations. Minimum 2 years of post-certification experience preferred. Primary care, assisted living, skilled nursing, geriatric, hospitalist, emergency department, or similar clinical experience preferred. Strong clinical assessment, communication, and organizational skills. Comfortable managing acute and chronic conditions in an older adult population. Ability to work independently and collaboratively as part of an interdisciplinary care team. #MP123
