Key Responsibilities

The Patient Navigator guides patients and families through their clinical episode of care to ensure seamless transitions and positive outcomes. They collaborate with multidisciplinary teams to manage patient education, psychosocial needs, and resource coordination.

Requirements

Candidates must have completed an approved nursing education program and hold a valid South Dakota Registered Nurse license. Basic Life Support (BLS) certification is required within 60 days of hire, and 1+ years of case management or patient care experience is preferred.
