Key Responsibilities
- Provides total case management and systematic reporting of progress, recommendations, and referrals in accordance with application guidelines.
- Actively participates in patient evaluation conferences with emphasis on discharge planning and referrals pertinent to continue rehabilitation and/or stabilization of clinical gains.
