As a Physician Assistant, or Nurse Practitioner, you'll be a vital member of our Interdisciplinary Team (IDT), providing compassionate primary care and helping create personalized care plans that support our participants' physical, emotional, and social well-being.

What You'll Do

*  Provide comprehensive primary care to participants

*  Complete assessments, evaluations, and care planning

*  Collaborate closely with nurses, therapists, social workers, and caregivers

*  Manage acute and chronic health conditions

*  Coordinate nursing home care and discharge planning

*  Order and review labs, diagnostics, and treatments

*  Educate participants and families on health management

*  Build lasting relationships with seniors and their caregivers

We're Looking For

Physician Assistant

* Graduate of an Accredited Physician Assistant Program

* Current North Carolina Certification

* DEA Registration

Nurse Practitioner

* Current RN and NP Licensure

* Prescriptive Authority Preferred

* DEA Registration

For All Candidates

* CPR Certification - will provide

* Minimum one year working with the frail or elderly population

* Two years of geriatric experience preferred

* Passion for serving older adults

* Team-oriented mindset

* Commitment to person-centered care
