Nurse Practitioner - Physician Assistant - NP - PA - No Weekends

Key Responsibilities

The provider will deliver primary care, perform assessments, and develop care plans for frail elders in collaboration with an interdisciplinary team. They are also responsible for managing acute and long-term illnesses, maintaining medical records, and educating participants and caregivers.

Requirements

Candidates must hold a bachelor's degree and possess a current North Carolina license as a Physician Assistant, Nurse Practitioner, or MD/DO. A minimum of one year of experience working with the frail or elderly population is required.
