Job Description

Physical Therapist (PT) – Mobile Outpatient | Middletown, NY | Full-Time, Part-Time & PRN

Stern at Home Therapy is seeking Full-Time, Part-Time, and PRN Physical Therapists (PTs) to provide individualized, one-on-one mobile outpatient therapy to patients throughout Middletown, NY, and surrounding Orange County communities.

Our mobile outpatient model lets therapists deliver personalized care in patients' homes while enjoying flexible scheduling, clinical autonomy, and dedicated administrative support.

Responsibilities

  • Conduct comprehensive physical therapy evaluations and develop individualized care plans.
  • Provide one-on-one physical therapy treatments to a primarily geriatric patient population.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide therapeutic exercise, gait training, transfer training, and fall-prevention interventions.
  • Recommend assistive devices and adaptive equipment when appropriate.
  • Educate patients and caregivers on home exercise programs, mobility strategies, and safety.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Complete accurate and timely clinical documentation.
  • Collaborate with physicians, caregivers, families, and interdisciplinary team members.
  • Coordinate with Stern's administrative team regarding scheduling and patient care.

Compensation

  • Full-Time – Minimum 16 Patients
  • Full-Time W-2 Salary
  • $92,000–$110,000 annually
  • Salary may increase based on patient volume.
  • Full-Time W-2 Tiered Per-Visit
  • $55–$70 per follow-up visit
  • $75 per 4-unit evaluation
  • Per-visit rates increase as patient volume grows.
  • Full-Time 1099 Tiered Per-Visit
  • $70–$80 per follow-up visit
  • $85 per evaluation
  • Annual earning potential up to $145,000
  • Per-visit rates increase as patient volume grows.
  • Part-Time – Minimum 10 Patients
  • Part-Time W-2
  • $65 per follow-up visit
  • $75 per 4-unit evaluation
  • Fixed per-visit compensation without a tiered structure.
  • Part-Time 1099
  • $72 per follow-up visit
  • $85 per evaluation
  • PRN – Minimum 5 Patients
  • PRN 1099 Only
  • $72 per follow-up visit
  • $85 per evaluation

Full-Time W-2 Benefits

Health Insurance And/or Medical Stipend

  • 401(k) with annual employer match starting at 1.5% and increasing up to 7%
  • 2 weeks of PTO after 6 months and a third week after 9 months
  • 5 paid holidays, available immediately, with employees choosing which holidays to observe
  • Student loan repayment assistance after the first year, starting at $125 per month
  • $500 annual continuing education (CEU) stipend
  • $250 monthly commuter stipend

Qualifications

  • Active Physical Therapist license in New York State required
  • Graduate of an accredited Physical Therapy program
  • Reliable transportation required to travel between patient locations
  • Strong clinical assessment, treatment planning, and documentation skills
  • Excellent communication and time-management abilities
  • Ability to work independently in a mobile outpatient setting
  • Experience with geriatric populations or mobile outpatient therapy preferred but not required
