Iowa Home Care is looking for compassionate, motivated PTs to join our team and provide quality therapy to patients in the comfort of their own homes. What You'll Do
As a Home Health Physical Therapist, you will provide individualized therapy based on each patient's goals, abilities, and needs.
Responsibilities Include
Evaluate patients in their homes and assess mobility, strength, balance, coordination, and functional limitations Create personalized therapy plans based on the patient's condition and goals Deliver therapeutic exercises and interventions to improve function and quality of life Provide gait, balance, transfer, mobility, and strengthening training Teach patients and caregivers strategies for safe movement and continued progress between visits Develop and instruct patients in appropriate home exercise programs Assess the need for assistive devices, adaptive equipment, and home safety modifications Track patient response to treatment and modify interventions as progress is made Maintain accurate and timely clinical documentation Identify and communicate changes in patient condition to the appropriate members of the care team Coordinate services with nurses, occupational therapists, speech therapists, social workers, and case managers Encourage patients to reach realistic functional goals while promoting independence and confidence Deliver care with professionalism, compassion, and respect for each patient's individual circumstances What We Value Professionalism We take pride in providing dependable, high-quality care.

Qualifications
Licensed to practice as a Physical Therapist in the state of Iowa. Minimum of two years of experience in physical therapy; community/home health experience preferred. Strong communication, organizational, and problem-solving skills. Valid driver’s license with reliable transportation.
Key Responsibilities
The physical therapist will evaluate patients in their homes to create and deliver personalized therapy plans focused on mobility, strength, and functional independence. They will also maintain accurate clinical documentation and coordinate care with an interdisciplinary team to ensure patient progress.
Requirements
Candidates must be licensed to practice as a Physical Therapist in the state of Iowa and possess a minimum of two years of experience. A valid driver's license and reliable transportation are required for home visits.
