Iowa Home Care is seeking a Physical Therapist Assistant to provide therapy services to patients in their homes throughout the Webster City and Fort Dodge areas.

In this role, you'll work under the direction of a licensed Physical Therapist to carry out individualized treatment plans and help patients improve their ability to move, function, and safely participate in everyday activities.

Home-based therapy gives you the opportunity to see the challenges patients face in their actual living environment and help them develop practical strategies for greater independence. Full-Time and Part-Time Opportunities Available

Your responsibilities will include: Provide physical therapy interventions in patients' homes according to the established plan of care Carry out therapeutic exercises and activities designed to improve strength, flexibility, balance, endurance, and functional mobility Assist patients with walking, transfers, positioning, and other functional movement activities Provide gait and balance training as directed by the supervising Physical Therapist Reinforce home exercise programs and teach patients and caregivers appropriate techniques Observe how patients respond to treatment and communicate progress or concerns to the supervising PT Identify and report changes in patient condition or functional status Help patients safely use prescribed assistive and adaptive equipment Reinforce strategies that promote fall prevention and a safe home environment Complete accurate and timely visit documentation Participate in communication and coordination with nurses, Physical Therapists, and other members of the care team Maintain professional boundaries while building positive relationships with patients and families Encourage patients throughout their rehabilitation journey and celebrate progress toward their individual goals Why Home-Based Therapy?

Qualifications

Qualifications: Graduate of an accredited Physical Therapist Assistant program Current Physical Therapist Assistant license in the State of Iowa Current CPR certification Valid driver's license, reliable transportation, and current auto insurance Home health experience preferred, but not required Strong communication, organizational, and clinical skills Ability to work independently while maintaining collaboration with the healthcare team

Key Responsibilities

The Physical Therapist Assistant will provide home-based therapy services by carrying out individualized treatment plans under the direction of a licensed Physical Therapist. Responsibilities include performing therapeutic exercises, gait and balance training, and documenting patient progress while coordinating with the interdisciplinary care team.

Requirements

Candidates must be graduates of an accredited Physical Therapist Assistant program and hold a current license in the State of Iowa. Additionally, a valid driver's license, reliable transportation, and current CPR certification are required.
