The Clinical Lead manages a defined patient panel, performing physical exams and formulating longitudinal care plans for patients with chronic conditions like CHF and CKD. They collaborate with interdisciplinary teams and external providers to ensure care alignment and improve patient outcomes while reducing hospitalizations.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in cardiology or nephrology. A current state-specific license, national board certification, and a valid driver's license are required for this role.
