San Luis Valley Health is seeking an Oncology Nurse Navigator (ONN).
This position assists patients in identifying goals and barriers to care and may assist patients and support systems with resolving financial, psychosocial, functional, and administrative issues by advising or options and/or referring to appropriate resources.
The ONN is engaged in the support and implementation of treatment plans, cancer care, and assisting with navigating financial assistance. The ONN will maintain clinical skill and support infusion center team when needs arise.
What You'll Need
- Bachelors or Masters degree, current RN license
- Chemo/Immunotherapy certificate (ONS/ONCC) required or within 6 months
- Basic Life Support certification required
- Oncology Nurse Certification (ONC) required or within 6 months
- Experience in conflict resolution
- Able to assist patients with financial, behavioral, and socio-economic issues
- Knowledge of Community Resources ability to navigate said resource
- Symptom management experience
- Knowledge of Oncology nursing/clinical skills
- Knowledge of Chemotherapy concepts and treatment plans
- Knowledge of survivorship care planning
What You'll Do
- Attend patient care meetings and provide feedback on patient needs.
- Demonstrate a sensitivity and responsiveness to a variety of cultural values and beliefs and social determinants of health.
- Conduct basic screenings of patients within the oncology hematology clinic, including review of barriers to health self-management; depression; stress; and substance abuse.
- Provide basic crisis intervention services to patients in need and preforms distress screening to assures that patients are having follow up appointments if the patient is in distress.
- Provide basic case management supports regarding health and mental health needs, functional abilities, readiness to change, caregiver stability, social supports, financial resources, environment and safety concerns, life care planning and self-management skills.
- Preform psychosocial coordination to assist patients with financial transportation, applications, patient assistance, or free drug programs. Assist with forms if needed if patient is unable to comprehend or has disabilities.
- Obtain prior authorizations for oral and intravenous chemotherapy regimens, funding coordination pharmacy coordination and assist with other authorizations if needed.
- Consistently follows evidence-based practices including quality core measure standards.
- Develops wellness services coordination team for monthly wellness groups, patient comfort measures, volunteer coordination and develops a resource library for patients.
- Must understand professional boundaries and appropriately refer diagnostic questions to Provider
- Coordinate with care managers or patient navigators in other settings as appropriate.
- Assists with WWC grant and keeps database current, attends necessary webinars for the grant.
- Participate in health team meetings to review individual and populations-based outcomes, patient experiences, and identify opportunities for practice improvement.

Requirements
Candidates must hold a Bachelor's or Master's degree and a current RN license. Additional requirements include Basic Life Support certification and the ability to obtain Oncology Nurse and Chemo/Immunotherapy certifications within six months.
