Job Details: Job Location: Crest Home Health and Hospice - Coeur D Alene, ID, Salary Range: Undisclosed, Speech Language Pathologist (SLP) – Crest Home Health & Hospice CrestHome Health & Hospice is looking for a compassionate and skilled Speech Language Pathologist (SLP) to join our home health team.

As a Home Health SLP, you’ll provide individualized speech and language therapy to patients in the comfort of their own homes. You’ll work closely with patients, families, physicians, and the home health team to help patients improve communication, maximize their independence, and reach their goals.

What You’ll Do Provide skilled speech-language pathology services in patients’ homes according to physician orders and individualized plans of care Complete initial and ongoing speech and language evaluations and develop appropriate treatment goals Provide therapy to improve or maximize patients’ communication, function, and quality of life Develop, implement, and update individualized plans of care Educate patients and family members on treatment strategies and home programs Monitor patient progress and communicate changes in condition or treatment needs with physicians and the care team Complete accurate and timely clinical documentation, progress notes, evaluations, and discharge summaries Participate in interdisciplinary care planning and collaborate with other home health professionals Complete required OASIS assessments and documentation as applicable Utilize appropriate ICD-10 coding and follow agency documentation requirements Participate in quality improvement and staff education activities as assigned What We’re Looking For Master’s or doctoral degree in Speech-Language Pathology from an accredited program Current Speech-Language Pathologist license in the state where services are provided Meets applicable state and federal requirements for practicing as an SLP Two years of experience preferred, but we encourage qualified candidates to apply Strong communication, organization, and clinical documentation skills Ability to work independently while collaborating effectively with patients, families, physicians, and the home health team A compassionate, patient-centered approach to care Why Home Health?
