Job Description

The Social Worker helps patients and families adjust to illness and life-style changes due to illness through counseling and coordination of community, social and hospital services, and intervenes appropriately in cases of suspected domestic violence or abuse.

Applicant must have Behavior Health experience, utilization and health insurance knowledge and able to work with patients 50 years and older. Able to complete social assessment, address referrals, and pre-cert new patients run family meeting and run groups.
